Here is the updated report and complaint I sent to the SC Attorney General Office of Consumer Affairs:

I ordered ATT U Verse Bundle in October 2013 with a promotional cost for the first year. As I was too far away from their main lines ATT offered DIRECT TV (rather than their standard TV portion) as part of the bundle. Short story is that after installation DIRECT TV would not resolve the issue of the $40 per month potion of the bundle for the first year. I called ATT and had a conference call January 7 and the ATT and DIRECT TV rep agreed that there was a problem supposedly due to the timing of their billing.

My email to DIRECT TV of March 3 2014....
Here is the summary: Order Date: 10/16/2013 DIRECTV Order Confirmation Number: 192568922

I have had nothing but trouble with your billing since I got this as part of
the ATT U Verse Bundle. I have sent you repeated emails that you fail to
give me the $40 monthly discount for the first year. You seem to think the
billing is direct and "gave" me $80 on account. Well the account is billed
through ATT and that is how it is paid, not to you directly. So the $80 is
useless to me and your representatives since the first part of February are
not at all resolving it.

The ONLY help was here:

I received my last combined ATT and Direct TV bill for $92.63 and paid it
even though the promotional credit for prior months of $80 has yet to be
applied.

The January bill of $135.58 was in error and did not include the promotional
discounts / credits.

RE the below:

Bill due DEC 26 was $87.83 and is OK (PAID)

Bill due JAN 26 was $135.58 way over charged. (PAID based on the below call
I made)
I called ATT JAN 7 when I received the inflated bill of $135.58 and Marissa
got Ben of Direct TV on line. Marissa: Said that when the 3 month special for the movie channels was removed the $10 Advanced Receiver credit and $30 Service rebate credit did
not kick in for the bundle special. IE: $40 monthly credit for first year bundle.

Ben of Direct TV joined and: Said the $10 and $30 did not kick in as it
should have. Has to do with the different cycles of Direct TV and ATT. Ben
did say that the next bill in February will have an $80 credit (for the JAN
$40 that did not kick in and the FEB regular $40 credit total). After that
beginning in March should have the $40 credit as it should have for the
remainder of the bundled special promotion.

END of the January 7 telephone conference.

Now DIRECT TV is charging me $380 for early termination per their email I received this evening (March 7 2014). They also threatened collection if I did not pay. I told them I would not pay it as they refused to resolve the billing error.
